Chico State Student-Athlete Medical Clearance

SportsWare: Medical Information System - Prior to participating on a team at Chico State, athletes must provide the Athletic Department with current address, emergency contact, medical alert, health history information, COVID-19 vaccine card and primary insurance information. (REQUIRED - A copy of both front and back sides of your COVID-19 vaccine card and insurance card. A cell phone photo of your card that you can upload works great.) To expedite this process, Chico State Athletic Training uses SportsWare, an online data entry system.
To log into SportsWare, visit www.swol123.net. On the SportsWare website choose: "SINGLE SIGN-IN BUTTON."

Next choose California State University Chico from the list.
Next you will be directed to your campus portal log in screen. Log in with your campus portal username and password.
Once logged into SportsWare please fill out everything in the "My Info" tab (marked with a red*) (personal info, emergency contact info, and insurance info and photo of your card). This can be found under the menu bar in the top left corner of the page. Please also submit a picture front and back of your medical insurance card.
Additionally, you will need to upload a picture of your vaccination card—front and back—under the "Forms" tab in your SportsWare account.

2. COVID-19 Requirements
For the 2022-2023 academic year, and per California State University system policy, all students, faculty and staff who will access campus facilities or participate in in-person learning or campus programs must:
- ​​​​​​B​​​​​​e up to date with their COVID-19 vaccinations as defined by CDC, and complete their vaccination certification by 8/1/2022, or
- Receive an approved medical or religious exemption by 8/1/2022, or
- Students, faculty and staff not accessing campus facilities or participating in in-person learning or campus programs are not required to be vaccinated but must complete their vaccination certification by selecting the "I do not plan to physically access CSU facilities…" option by 8/1/2022. Â

COVID-19 Vaccination Requirement for Chico State Intercollegiate Student-Athletes:
Chico State Intercollegiate Athletics requires all new and continuing student-athletes to comply with the CSU COVID-19 vaccination policy as a condition of medical clearance for athletic participation on all intercollegiate athletic teams. The vaccination obligation for student-athletes is separate and distinct from the campus vaccine certification process.
New Student-Athletes must:
- Submit an up to date COVD-19 vaccination card through the SportsWare Medical information System as part of the athletics medical clearance process.

OR
- Receive an approved medical or religious exemption from the respective campus office.
Continuing student-athletes:
All continuing student-athletes that were on active intercollegiate athletic team rosters at the conclusion of Spring 2022 have cleared the vaccination requirement.

Non-compliance with CSU COVID-19 vaccination requirement:
Student-athletes who self-certify "I do not plan to physically access CSU facilities" or who fail to comply with the COVID-19 vaccination requirement will not be cleared for participation in intercollegiate athletics.
3. Pre-participation physical Exam
Pre-participation Athletic Physical Form - Required for all NEW student-athletes
1. Present a copy of a recent pre-participation physical from your family physician (MD) to the athletic training staff. YOU MAY USE THIS FORM (PDF). NCAA rules require that the examination must have been administered within six months prior to participation. 2. If you are unable to complete the pre-participation physical at home, one will be arranged for you at the Student Health Center. You must contact the athletic training staff prior to going to the Student Health Center.
4. Orthopedic Exam
Required for all student-athletes, every year. This in-person exam must be scheduled with the athletic training staff.
5. Satisfy the NCAA Sickle Cell Testing Requirement
Per NCAA rules, all student-athletes must comply with one of following steps regarding the NCAA Sickle Cell Testing Requirement. No student-athlete will be cleared to participate in any activity until this requirement is met. Read more detailed information regarding the NCAA Sickle Cell Trait Policy. In accordance with the NCAA recommendation, we encourage all students to know their Sickle Cell Trait status. Knowing your Sickle Cell Trait status is helpful information for you and the Sports Medicine staff.

There are 2 options to fulfill the Sickle Cell Trait testing requirements:
1. Provide test results of prior Sickle Cell Trait testing. Most individuals born after 1990 in the United States were tested at birth. You can request copy of this test result from your pediatrician. Also, if you were born in California you can request your results from California Department of Public Health Newborn Screening Program. COMPLETE THE PDF ON THEIR WEBSITE LINKED HERE. This can take up to 30 days for the results to be received.Â
2. Take a new blood test for Sickle Cell Trait and provide documentation to the athletic training staff in your Sportsware account. (This can be performed through your family physician or through the Student Health Center). Please contact the athletic training staff prior to going to the Student Health Center for this referral.
6. Baseline Concussion Testing
Baseline concussion testing will be administered with SWAY (a mobile app on your phone). The SWAY baseline concussion testing will be performed by one the athletic training staff at the time of your orthopedic exam.
7. Banned Prescription Medications (treatment of ADD/ADHD)
The NCAA bans performance enhancing drugs to protect student-athlete health and safety and ensure a level playing field. The NCAA also recognizes that some of these substances may be legitimately used as medications to treat student-athletes with learning disabilities and other medical conditions.
Accordingly, the NCAA allows exceptions to be made for those student-athletes with a documented medical history demonstrating the need for regular use of such a drug.
Exceptions may be granted for the following classes of banned drugs: stimulants, beta blockers, diuretics, antiestrogens, anabolic agents, and peptide hormones.
Student-athletes taking medications to treat ADD/ADHD will be particularly affected. The NCAA requires student-athletes taking stimulant medications for the treatment of ADD/ADHD to provide adequate documentation of diagnosis and treatment plan to qualify for a medical exception. Therefore, if you are taking any medications banned by the NCAA you MUST provide documentation from the treating physician, outlining in specific detail the diagnosis, treatment plan and medications, to the athletic training staff PRIOR to participation in your chosen sport. Failure to comply with this requirement could keep you from participating in your sport for up to one year should you have a positive drug test, as a result of the NCAA's Year Round Drug Testing Program.
